Healthcare and elective residence: how assistance works for those relocating to Italy

Living in Italy with an elective residence visa is a fascinating choice, but it also involves certain bureaucratic complexities. One of the most frequent concerns is access to public healthcare: is it possible? And how does it work?

Healthcare for those moving to Italy with this type of visa follows different rules than those applied to workers or students and requires careful planning.

This guide is designed to offer practical, clear, and up-to-date information about the obligations, rights, and timelines related to healthcare assistance for foreigners in Italy with elective residence.

Who is entitled to healthcare and what the law says

Those who obtain elective residence are not automatically entitled to access the Italian public healthcare system. The reason is that the visa is not linked to employment, so automatic enrollment does not apply. However, the law allows for voluntary enrollment in the national health system by paying an annual contribution.

In practice:

  • Enrollment is optional but highly recommended
  • It is completed at the local ASL (health authority) in the municipality of residence
  • It requires payment of a yearly fee established by the region

Those who do not enroll must rely exclusively on private health insurance, which is often more expensive and offers fewer services.

How to register with the national health system (SSN)

SSN registration for elective residence holders is a straightforward administrative process, but the correct documents must be submitted to the local ASL. Required documents include:

  • Residence permit or proof of application
  • Valid passport
  • Italian tax code
  • Official certificate of residence
  • Proof of payment of the healthcare contribution

After registration, a foreigner’s health card is issued, allowing full access to services guaranteed to Italian citizens.

When is the health card issued?

The health card is sent by mail within a few weeks. In the meantime, the ASL provides a temporary certificate that allows immediate access to healthcare services.

How much does enrollment cost and what does it include?

The cost of SSN enrollment for elective residence holders is based on a fixed contribution, which may vary slightly by region but generally amounts to around €2,000 per year.

Voluntary enrollment grants access to all services provided by the SSN across the country, on equal terms with Italian citizens, including the ability to choose a general practitioner.

It is important to note that enrollment lasts for a calendar year (from January 1 to December 31), is non-refundable and cannot be retroactively applied.

The health card for non-EU citizens guarantees:

  • Access to a general practitioner (family doctor)
  • Specialist visits and diagnostic services
  • Hospitalization in public or affiliated facilities
  • Vaccinations, prevention programs, and screenings
  • Medications covered by the national system, based on classification
  • Emergency medical assistance

Is registration required to obtain a residence permit?

It is not mandatory for all visa types, but in the case of elective residence, some immigration offices require proof of healthcare coverage, either through the SSN or an equivalent private policy, for the issuance or renewal of the residence permit.

What happens if you don’t enroll in the SSN?

Without enrollment in the national health system, a foreigner with elective residence has no access to public healthcare. This can be risky, especially in the case of medical emergencies or chronic care.

Additionally, some hospitals may require full payment for services if you are not covered.

Many foreigners only discover after arriving that SSN enrollment is not automatic and must be explicitly requested. Knowing the procedures and costs in advance helps avoid future issues and allows full enjoyment of the healthcare rights linked to elective residence.

Facing the connection between healthcare and elective residence with awareness is essential for anyone choosing Italy as their new home. Properly organizing SSN registration, obtaining the foreigner’s health card, and accessing quality healthcare are central to building a stable and secure life in our country.

Each region may apply slightly different rules, and each case can present unique circumstances. For this reason, relying on experienced professionals can make a significant difference.
